Diabetes can require constant monitoring. During home health visits, our team will educate you on your diet and medication management as we monitor your blood sugar and help you manage your insulin.

  • Prevention and detection of complications

Neurological diseases

Our providers are experienced in working with individuals who have neurological conditions such as Parkinson’s disease, dementia, and others.

We will work with you and your loved one to address the following:

  • Medication management
  • Cognitive behavioral therapy
  • Assess the home for fall risks
  • Assess and treat behavioral, psychological, and physical symptoms
  • Occupational therapy
  • Caregiver training for loved ones
